U18s must have parent/guardian consent to attend.
I have read, understood and consent:
I give permission for my child to attend Weston HC Awards night.
I understand that persons from outside of Weston HC will also be in attendance.
I understand that alcohol will be served at this event and that only over-18s, identified by wristbands, are permitted to consume it.
I understand that my child needs to be picked up promptly at the end of the event.
I am responsible for arranging transport to and from the venue. Prompt pick-up at 11.00pm.
If my child behaves in a way that puts themselves or others at risk, I may be contacted to collect them from the event.
